A few must-haves for this position:
Current license as an Licensed Practical Nurse (LPN) in the State of MN. License is active and in good standing.
2 years' experience as an LPN.
Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance.
CPR Certification
Some functions of what you would do as an LPN:
Participate in ongoing interdisciplinary assessment of the patient.
Participate in the development and evaluation of the plan of care in partnership with the patient, representative, and caregiver(s).
Provides services that are ordered by the physician as indicated in the plan of care.
Recognizes and reports changes in the patient s condition to the RN.
Counsels and educates the patient and family in meeting healthcare related needs.
Completes timely and accurate clinical notes including addressing patient s progress.
