Registered Nurse- Quality Safety Value Patient Safety Manager

Functions as the subject matter expert and organizational leader for the Minneapolis VA Patient Safety Program. Provides leadership and utilizes the nursing process in the assessment, development, provision and evaluation of patient safety initiatives. Supports a culture of safety by promoting transparency, blame-free reporting of safety concerns, and encouraging/leading collaboration across services and disciplines to seek solutions to patient safety problems. Ensure patient safety program is compliant with the National Center for Patient Safety Patient Safety Handbook and other internal/external requirements (VISN, TJC, OIG). Directs and manages the implementation of an integrated Patient Safety Program that supports the VHA mission, goals, objectives, policies, and procedures. Coordinates Root Cause Analysis (RCA) activities; serves as advisor for all RCA' facilitators, including Aggregate Reviews and HFMEAs; tracks all RCA action plans to closure, and reports status to Executive Leadership. Recommends changes in practice based on the analysis and recommendations from root cause analysis findings. Manages and utilizes a variety of automated/software systems and supporting processes for Patient Safety (including but not limited to):
Joint Patient Safety Reporting System (JPSR), the National Center for Patient Safety Web SPOT program, and a variety of other computerized patient record system (CPRS), and Vista extract reports. Actively chairs the Patient Safety Committee and represents Patient Safety or QSV on various MVAHCS, VISN, and VHA councils, committees, and interdisciplinary teams as assigned. Functions as a consultant to others in analyzing and addressing patient safety communication problems. Analyzes data associated with current patient safety initiatives and develops strategies to implement changes to meet evolving needs. Reviews and evaluates all Patient Safety Reports (PSR), recommends appropriate action based on the VHA Patient Safety Improvement Handbook, completes appropriate follow-up and brings all reported Patient Safety events to closure. Tracks, aggregates, and disseminates Patient Safety events to improve organizational safety, educate all staff, and promote a highly reliable organization. Manages and responds to alerts/advisories from the National Center for Patient Safety Database, including timely notification, resolution and documentation of same. Work Schedule:

The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia by the time of employment. BSN preferred. Master's preparation in Nursing or related field preferred. 3-5 years of professional nursing experience preferred. Advanced Lean training preferred. Grade Determinations:

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. The VA Medical Center is a major interdisciplinary teaching facility, serving medical, surgical, and mental health inpatients and outpatients. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Must be able to lift at least 45 pounds. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. This position requires heavy lifting, 45 pounds heavy carrying, 45 pounds straight pulling (4 hours); pushing (4 hours); use of fingers; both hands required; walking & standing 8-12 hours; repeated bending 12 hours; both legs required; ability for rapid mental and muscular coordination simultaneously; near vision correctable @ 13 to 16 to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; depth perception; ability to distinguish shades of colors; hearing (aide permitted). The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
