Position
Summary:

We're seeking a Physician (MD/DO) who is energized by the idea of working in diverse senior settings--from assisted living to memory care--and building long-standing trust with residents. If you're looking for a flexible, mobile, and mission-driven care role, this is it.

Key
Responsibilities:

• Round at assigned senior living communities to deliver primary care services
• Provide medical oversight, diagnostics, and treatment planning
• Engage in chronic care management and reduce unnecessary ER visits
• Act as a clinical leader within an interdisciplinary support team
• Ensure thorough documentation and follow-up through EMR

Qualifications:

• Active Minnesota MD or DO license
• Experience in geriatric medicine, internal medicine, or primary care
• Ability to work independently while collaborating with facility teams
• Valid driver's license and reliable transportation
